Service Planning Area 3, or SPA 3, serves the communities of Alhambra, Altadena, Arcadia, Azusa, Baldwin Park, Claremont, Covina, Diamond Bar, Duarte, El Monte, Glendora, Irwindale, Monrovia, Monterey Park, Pasadena, Pomona, San Dimas, San Gabriel, San Marino, Temple City, Walnut, West Covina, and others.
Pomona Wellness Community
The Pomona Wellness Community offers a place for community members to gather, find resources, or participate in wellness activities. At the Wellness Community, there are a variety of activities such as physical activity classes, nutrition and health resources, and other support resources. San Gabriel Valley Hospital Collaborative
The Hospital Collaborative is an initiative of and facilitated by the Health Consortium of Greater San Gabriel Valley (Health Consortium). The mission of the Health Consortium is to strengthen the health care safety net and optimize seamless access to high quality physical health, mental health and substance use disorder (SUD) services in the Greater San Gabriel Valley (SGV).
